How to assign a questionnaire to a patient

1. Choose a patient you’re sending an assessment to.
2. Click on Start Protocol

3. Choose how you want to send out an assessment.

• Administer on Device is when a patient is taking the assessment on the actual device being used - this is the most common option when administering in-office 
• Copy Link will generate a unique url that will be copied to your clipboard. You can then use this url to paste in an email that you compile, or in a chat, text or other related medium
• Send via Email will send out the assessment via [e-mail] and will have the options to enable reminders for your patient to complete the assessment

4. Click on Administer Your Own Protocol

5. Click on Protocol

6. Click on Create a One-Time Protocol +

7. Choose the assessment you want to send (either a cognitive assessment, or a questionnaire, or both)

8. To include a Questionnaire, click on Questionnaires. For a full list of questionnaires we offer, click here. 

9. Click on Cognition to choose which tasks you want to send to your patient

10. When ready, click on Next

11. Confirm your details and then click Next

12. Toggle on the reminders if preferred then click Next

13. Customize your wording or if ready, click on Send
