How to administer an assessment (tasks and questionnaires) to a patient

Follow the steps below or watch the video to send cognitive tasks and/or questionnaires to your patients.

1. Choose a patient: Start by selecting the patient to send the assessment to.

2. Click "Start Protocol"

3. Choose how to send the assessment: You have a few options-

• Administer on Device: Use this when your patient will take the assessment directly on the tablet, laptop, or desktop in front of them. This is the most common in-office option.
• Copy Link: This generates a unique link that will be copied to your clipboard and can be pasted into an email, text, or chat being sent to your patient. 
• Send via Email: The assessment will be sent to the email address included in the patient's profile from [e-mail]. You can also turn on automated reminders to help your patient complete it.

4. Click "Administer Your Own Protocol"

5. Click on the "Search Protocols" bar. Then click "Create a One-Time Protocol"

6. Choose what to include

You can send cognitive tasks, a questionnaire, or both.

• To include a Questionnaire, click on "Questionnaires".
  ➤ View the list of questionnaires available. 

• To include Cognition tasks, click on "Cognition" and choose the tasks you’d like the patient to complete.

7. Click "Next" and confirm the protocol details 

Click "Next" again to confirm. 

8. Add Reminders (Optional)

Checkmark the boxes to set automated follow-up emails.

9. Customize the Message (Optional)

Personalize the email message if you'd like—or, just click "Send" when you're ready.
